The College Career Connection provides training and technical assistance to postsecondary education partnerships focused on providing an inclusive postsecondary education to students with severe disabilities. Currently supporting six partnerships in Massachusetts, the postsecondary model is designed to improve postsecondary education and employment opportunities for students with severe disabilities. It takes into account unique aspects of each student including aspirations, family wishes, and cultural background. The C3 model incorporates the practices of dual enrollment and supported education for students who typically stay in high school into their early 20s.

The College Career Connection includes creating student vision and goals, developing flexible services and supports, assisting students to access college and employment, implementing ongoing services and supports, and evaluating outcomes.
