The Transitions program focuses on building academic skills, life skills and general independence for high school graduates and young adults with autism and learning differences. Its specialty isn’t just in teaching students how to study or learn a trade – it’s in showing its students that they can live a life that even they never dreamed of. Through partnership with local colleges, internships in community businesses and supported living opportunities, Transitions helps young people on the autism spectrum or with learning differences make successful transitions from adolescence to young adulthood. Students have access to state-of-the-art facilities, an experienced staff and an evidence-based curriculum that provides individualized attention and training in skills students will use for the rest of their lives. Transitions gives students the tools and supports they need to learn, thrive, and live happy and productive lives that they and their parents hoped for, but never thought possible. In addition to FMCC, we also partner with SUNY Albany, SUNY Cobleskill and SCAD.

It is a 4 year Life and Career Studies Program designed for young adults with autism and other learning disabilities to learn employment and life skills needed to be independent. College level courses on employment and career skills are a part of the 4 year program. Students may also enroll in colleges and we will support them with their college goals. We partner with FMCC, SUNY Cobleskill and Albany State University where we have physical program space as well as students enrolled in college courses in those locations. |

During the summer months we offer continued education in all of the core areas of study: Career Success and Employment, including paid and unpaid internships Independent Living Skills Advocacy Leadership Residential Life Wellness We also offer week long speciality camps that take a closer hands on look at things like life after school, employment, STEAM careers, independent living and social skills. |

Students that are enrolled in the program have a choice to study either a college or a career track. College track students will enroll full or part time in a local university of their choice. Career track students will take college level Career Success classes that are credit bearing but not part of a matriculated course of study. Career track students focus more on employment than college based outcomes. |

Each student is assessed at time of admission and annually and matched up with an internship experience. Most are unpaid but a few are paid opportunities. Each student is required to have at least 250 internship hours as part of our agreement with FMCC and student's attendance and participation in the 3 career classes that are a part of the FMCC curriculum. In the student's junior year the student is connected with ACCES VR for assistance finding a competitive community job and they are assigned a Job Developer and a Job Coach through our agency supported employment program Employment Opportunities. |
