Program Sustainability Assessment Tool

The Program Sustainability Assessment Tool is a 40 item self-assessment that program staff and interested groups can take to evaluate the sustainability capacity of a program. When you take the assessment online you will receive an automated summary report of your overall sustainability. You can use these results to engage in sustainability planning. The assessment is made up of 40 multiple choice questions. You will rate your program/coalition/set of activities across the 8 sustainability components. The assessment takes about 10-15 minutes to complete. The assessment can be used by programs at the community, state, and national level. While the assessment was designed for use by public health programs, we believe it is also relevant for college programs assessing sustainabilty plans.
